Dapoxetine 90 mg (e.g., in generics like Poxet 90, Vriligy 90) is a medication containing an ultra-high dose of Dapoxetine Hydrochloride. This dosage is three times the standard starting dose (30 mg) and 1.5 times the maximum recommended daily dose (60 mg).
It is crucial to understand that a Dapoxetine 90 mg tablet is categorically not intended to be taken whole in a single dose. Its primary purpose is to provide patients with an economical way to obtain three standard doses (30 mg) by splitting the tablet.
Dapoxetine is the only active ingredient specifically designed and clinically approved for the treatment of premature ejaculation (PE) in men aged 18 to 64. It does not treat erectile dysfunction (ED) and does not affect erection hardness.
Standard Starting Dose: 30 mg.
Maximum Daily Dose: 60 mg.
Taking 90 mg of Dapoxetine at once carries an extremely high risk of serious side effects, especially fainting (syncope).
The mechanism of action of Dapoxetine is centered on the neurochemical processes in the central nervous system.
Serotonin: Ejaculation is controlled by reflexes where serotonin (5-HT) plays a key inhibitory role.
Action of Dapoxetine: Dapoxetine is an ultra-short-acting Selective Serotonin Reuptake Inhibitor (SSRI). It temporarily increases the concentration of serotonin in the brain.
Result: This enhances the inhibitory effect on the ejaculation centers, allowing the man to significantly prolong sexual intercourse (by 3-4 times).
Taking such a high dose requires extreme caution.
Initial Use: It is strictly forbidden to start treatment with 90 mg. Always start with 1/3 of a tablet (30 mg).
Standard Maximum Dose: 2/3 of a tablet (60 mg).
Maximum Daily Dose: Do not take more than 60 mg (2/3 of a tablet) within 24 hours.
Method: Swallow the required portion of the tablet with a full glass (at least 250 ml) of water.
Timing: Take 1–3 hours before planned sexual activity.
Alcohol: ABSOLUTE PROHIBITION. Combining Dapoxetine (especially at high doses) with alcohol manifold increases the risk of neurocognitive side effects (dizziness, drowsiness) and can lead to fainting (syncope).
The list of contraindications related to the drug's neurochemical action is very strict.
Cardiovascular Disorders: Cardiac rhythm disturbances (blocks, bradycardia), severe heart failure, IHD.
Hepatic Impairment: Severe and moderate severity.
Psychiatric Disorders: History of mania, severe depression, uncontrolled bipolar disorder.
Drug Interactions: Absolutely forbidden to take with:
MAO Inhibitors (MAOIs): Risk of serotonin syndrome.
Potent CYP3A4 Inhibitors: (Ketoconazole, Erythromycin, Grapefruit juice).
Other SSRIs: (Antidepressants).
At the 90 mg dose (a whole tablet), side effects are very frequent and can be very pronounced.
Very Common: Severe nausea, severe dizziness, headache.
High Risk: Fainting (syncope) due to orthostatic hypotension.
Splitting is the Key to Safety: Dapoxetine 90 mg is ideal for splitting. By dividing it into three parts, you get three 30 mg starting doses. This is safe and economical.
Do not chase the dose: If 60 mg of Dapoxetine provides a sufficient effect, switching to 90 mg will not provide a significant increase in efficacy but will sharply increase the risk of adverse reactions.
Take with water, while sitting: To minimize the risk of fainting, always take the drug while standing or sitting, washing it down with a full glass of water, and avoid standing up abruptly.
How long does Dapoxetine 90 mg last? The duration of Dapoxetine's action does not depend on the dose. The ejaculation control effect lasts for 4-6 hours.
Can I take 90 mg if 60 mg did not work? This is possible only after consulting a doctor and provided you have excellent tolerability of 60 mg. Increasing the dose to 90 mg on your own is dangerous.
How does Dapoxetine differ from delay sprays (Lidocaine)? Dapoxetine (pills) works "from the inside" via the brain, maintaining full sensitivity. Sprays (Lidocaine) work "from the outside," causing numbness and mechanically reducing sensitivity.
