  • Mechanism of Action: This drug is not absorbed into the bloodstream. It acts locally in the intestine, blocking the enzymes (lipases) that break down fats. As a result, about 30% of the fat from food is not absorbed and is excreted in the feces.

  • Efficacy: Moderate. It only works if your diet contains fat.

  • Risks: Very unpleasant side effects—oily spotting, flatulence, fecal urgency, steatorrhea (fatty stools). Requires simultaneous intake of fat-soluble vitamins (A, D, E, K).

3. Central-Acting Combination Drugs

  • Active Ingredients: Naltrexone + Bupropion.

  • Mechanism of Action: Acts directly on the pleasure and hunger centers in the brain. Bupropion (an antidepressant) reduces appetite, and Naltrexone (an opioid antagonist) reduces food cravings and the pleasure derived from eating.

  • Efficacy: Moderate, but helps combat compulsive overeating.

  • Risks: Psychiatric side effects, nausea, increased blood pressure.

Category 2: Dietary Supplements & "Natural" Remedies (Over-the-Counter)

1. "Fat Burners" (Thermogenics)

  • Active Ingredients: Caffeine, Green Tea Extract, Synephrine (from bitter orange), Guarana.

  • Mechanism of Action: Slightly speed up metabolism (thermogenesis) and stimulate the central nervous system, providing more energy for workouts.

  • Efficacy: Very low without diet and exercise. They help burn an extra 50-100 kcal per day.

  • Risks: Insomnia, anxiety, tachycardia, increased blood pressure.

2. Appetite Suppressants (Herbal)

  • Active Ingredients: Garcinia Cambogia (Hydroxycitric Acid), Hoodia Gordonii, Glucomannan (Konjac).

  • Mechanism of Action: Glucomannan is a fiber that swells in the stomach, creating a mechanical feeling of fullness. Garcinia and Hoodia supposedly affect serotonin, but their efficacy is not clinically proven.

  • Efficacy: Very low to non-existent.

3. Fat Metabolism Stimulators

  • Risks: Banned in most countries (including the EU, USA, and Ukraine) due to catastrophic cardiovascular risks. It causes severe tachycardia, heart attacks, strokes, and psychiatric disorders.

  • The Problem: It is often a "secret" ingredient in counterfeit supplements.

2. Diuretics and Laxatives

  • Mechanism of Action: They do not burn fat. They remove water (diuretics) or bowel contents (laxatives).

  • Efficacy: They provide a rapid but false weight loss on the scale (up to 2-3 kg per day).

  • Risks: Dehydration, electrolyte imbalance (loss of potassium and magnesium), leading to arrhythmia, seizures, and kidney failure.

Professional Advice and Conclusion

As a medical professional, I cannot recommend taking any weight loss medication without an in-person examination.

  1. Start with a Doctor: The first step is to visit an endocrinologist and a nutritionist. It is necessary to rule out medical causes for weight gain (e.g., hypothyroidism, insulin resistance, PCOS in women).

  2. Lifestyle is the Foundation: No pill will ever replace a caloric deficit, balanced nutrition (protein, fats, carbs, fiber), and regular physical activity.

  3. Safety First: Never buy weight loss products with an unknown composition or those that promise ultra-fast results. It is almost always a scam containing diuretics or banned sibutramine.

  4. Indian Generics: If we are talking about generics, the only safe options (when prescribed by a doctor) are pharmaceutical-grade drugs like Orlistat. Buying "super fat burners" online is playing Russian roulette with your health.
